Forms

The School Ministry emphasizes reflection and intentionality, not standardization and busy work.
To that end, we've developed some streamlined forms to help students think through various processes and best capture insights through self-reflection.
These are required for credit and include:


  • This Book Study Form, as a means to capture key points and move from comprehension to life application. Upon completion of a "self-study" book, students will download this word version, respond to the prompts, and return a completed version to their advisor for credit.

  • This Ministry Identification Form as a "pre-assessment" exercise to formulate growth goals and desired learning outcomes before further embarking in ministry. Prior to investing significant time in a ministry, students will download this word version, provide responses to reflection questions, and work with their mentor and/or advisor to get plugged into ministry opportunities that best fit the desired objectives.   • This Mentorship Pairing Form that solicits input from the student on planned focus areas, important themes around their season of life, and desired growth and character goals. Students will download this word version, capturing preferences and objectives, and return a completed copy to their advisor to get paired with a suitable mentoring arrangement.

  • Once paired with your mentor (above bullet), the student and mentor will individually read this Mentorship Guidance before meeting and then jointly fill out a Mentorship Agreement at their first meeting. Prior to the first meeting, the student will also complete the "pre-meeting" sections of the "Long Form" and send this to their mentor. Students and mentors will generally meet monthly with "longer meetings" take place once per quarter using the "Long Form" and "shorter touchpoints" take place twice per quarter using the "Short Form". The student is responsible for initiating meetings and submitting the forms with mentor input. Enjoy!

  • This Missions Community Reflections Form that helps a student distill key insights and apply them to their own potential avenues. After attending an impactful Missions Community meeting, students will download this word version, record their reflections and thoughts, and discuss with their mentor and/or advisor to solidify personal takeaways for their own potential path.
